Chapter Member Status Change Form

This form should be completed within 48 hours of a verified membership status change.

Status Definitions

Active: A formally initiated member who pays dues, and participates in day-to-day activities of the chapter
On-Status: A member who is still apart of your organization, still has the right to wear their Greek letters, but no longer participates in the day-to-day activities, and may no longer be paying dues, depending on Chapter Bylaws
Termination: An initiated member who has been removed from active membership by the organization and no longer has any ties to the organization
Self-Termination: An initiated member who decided to terminate their collegiate affiliation and no longer have any ties to the organization
New Member - Withdraw: A member who withdraws from their organization's new member process before the initiation process
New Member - Termination: A member who was removed from the new member process by their organization before the initiation process
Transfer: A member who is no longer attending GVSU
Alumni: A member who has graduated from GVSU
