Health Compliance

The Purpose

Students, faculty, staff and volunteers (associates) at Grand Valley State University (GVSU) may participate in clinical or non-clinical experiential education or research at health-related affiliated organizations. In order to be protective of everyone involved, associates and the community served, certain participation guidelines have been established. Infectious communicable diseases may be present in affiliated organizations. During experiential education or research activities, associates may interact with or be exposed to clients who are immunocompromised, or with diseases such as, but not limited to, tuberculosis (TB), hepatitis B, influenza, and other infectious conditions. University policy, state and federal statutory regulations, and accreditation standards for academic programs and affiliated organizations require that our associates comply with certain health, safety, and legal requirements. As such, GVSU is contractually mandated to ensure all associates attain and maintain full compliance as defined in its affiliation agreements.

Students will be required to have certain Immunizations (outlined below), a Physical Examination, a required Criminal Background Check and Drug Screen, obtaining/maintaining Cardiopulmonary Resuscitation Certification, completion of online written tests (HIPPA, Elder Justice, Tuberculosis and Infection Control, Bloodborne Pathogens Training), Student Authorization to Receive and Release Information, and Technical Standards Acknowledgement.  Students admitted into the GVSU MAT program will be immersed into the health care setting throughout their education and are required to follow the GVSU Health Compliance Policy.  Prior to the start of their first clinical experience students must have all documents approved by GVSU Health Compliance Office.  It is strongly encouraged that you have your immunizations completed prior to admittance to GVSU MAT program.*

*GVSU students will be required to fulfill the immunizations and physical examination requirements to complete observation hours required in ATH 210, prior to applying to GVSU MAT Program.  GVSU students will need to complete the remaining requirements mentioned above prior to the start of the MAT Program.


Health Compliance FAQ

Please scroll down to the bottom of this site, download the document and have your health care provider fill it out.  You must have them completed GVSU documents in addition to any other paperwork your health care provider gives you.

  • Measles, Mumps, and Rubella - If less than 2 immunization dates – titer results must be submitted
  • Tetanus and Diphtheria or Tetanus, Diphtheria and Acellular Pertussis - last immunization must be within 10 years
  • Hepatitis B immunizations dates (series of 3 immunizations) AND Hepatitis B surface antibody titer result - NOT antigen
  • Varicella - If less than 2 immunization dates or documentation of disease - titer result must be submitted
  • Influenza -  last immunization must be within 1 year
  • Meningococcal - NOT required for AT students
  • Tuberculosis (TB) - A two-step TB skin test or QuantiFERON-Gold/T-Spot test is required, unless history of a positive tuberculin test. The first TB skin test must be documented as “negative” and that it was completed within the past 12 months, the second TB skin test must be completed no earlier than 7 days after the first.

Most of the immunization titers will take about one week to complete.  A few will take longer than this.

  1. TB testing - if you have never been tested or it has been longer than 12 months this is a two step process then.  
    • Get test - wait two days - read test - wait 7 days - get second test - wait two days - read second test.
  2. Hepatitis B testing - many end up not immune even though they have been vaccinated.
    • Physician may chose one of two routes for negative titer
      • Booster wait 4-8 weeks followed by repeat titer - usually immune, but not always
        • If not then will need second booster and repeat titer
      • Repeat whole series
        • First shot - 1 month wait - 2 shot - 6 month wait - 3 shot - titer again

You will need to have a copy of all immunization records and titer results.

A titer (pronounced = t - long i - ter) is a blood test that checks your immune status to a vaccination or disease you may have had in the past.  There are two types of immunity a person can have:

  1. Active Immunity
    1. The body’s immune system produces antibodies and cellular immunity which usually lasts for many years to a lifetime.
    2. Develops from surviving infection or by vaccination.
    3. Every individual varies in response and production of antibodies.
  2. Passive Immunity
    1. Immunity transferred from one to another
    2. Develops from mother to infant or by blood product such as immune globulin.
    3. This immunity usually only offers temporary protection during a period of weeks to months.

Most titers take a minimum of one week to obtain results. There is usually up to an additional week for the Health Compliance Office to process once you have submitted to them.

If you have had a physical within the last year, go back to that physician and see if they will fill out the Health and Immunization form.  If not you will need to schedule another physical.  Physicals must be within the last 12 months. See the document below.

  1. Yes - You are Health Compliant - You have all your paper work completed and have no more testing to be done - your immune to all required immunizations listed
  2. No - You have not started or completed any of the health compliance paperwork or immunization
  3. I am in process of obtaining the proper immunizations - You have everything completed however, you are still waiting on finishing the following
    1. Hep B - repeat series or booster titer result, but prior to the first day of class you will be complete
    2. Waiting on TB second test
    3. If you are waiting on anything else, you should answer no

This depends on each individual insurance coverage.

    • Metro Health in Allendale (by Family Fare).  This is closest to GVSU Allendale Campus but there are several other locations in the Grand Rapids area.

That's right, GVSU Health Compliance Office requires just Cardiopulmonary Resuscitation.  For accreditation and as an athletic trainer, you will be required to have current CPR/AED and First Aid certifications.  

For your ATCAS application, you will need electronic copies or a photocopy of the front and back of your card(s).  Certifications must be from American Red Cross or American Heart Association.  CPR/AED cards must be the basic life support certification.  First Aid cards must be the equivalent of Responding to Emergencies.  Any other certifications will not be accepted (e.g. lifeguard). The course must have hands-on participation - no online-only course accepted.

Both organizations certifications will be valid for 2 years.

Nowhere for now.  Once admitted into the GVSU MAT program you will receive further information regarding when and where to go.

Hold on to them for now.  Once admitted to GVSU MAT Program, you will upload your documents into CastleBranch.  The university uses CastleBranch which has a $35 dollar one-time fee for portal access. If you are already a user of CastleBranch we will ask to have your tracker changed. For more information please go to the GVSU Health Compliance website.

Once admitted into the program, the only items that will be need repeating are as follows:

  • TB - once every 12 months
  • Influenza - once every 12 months
  • Tetanus - depending on when you received your last one, once every 10 years
  • First Aid and CPR/AED - depending on when you were last certified, once every 2 years

**Keep copies of all records - future employers will require this same information - if you lose it you will need to repeat it**
