Music, Theatre, and Dance Major Declaration Form
Students
Please fill out all information using your GVSU account.
The system will automatically forward the email to your
selected advisor. Once the advisor has approved your change
of major, it will be forwarded to the department chair, who will
review, and if approved, send the application on to the Registrar,
copying you and your advisor. If the change of major does not show up
in MyPath after that email, you may use the department chair’s email
to the Registrar when contacting the Registrar to inquire about your
major change.

Please review the student’s submitted information. If approved,
please forward that email to the department chair.
