Club Sports Practice and Home Event Request Form 2023-2024

Preferred practice days/times are not guaranteed, but we will do our best to accommodate requests. Clubs must be in good standing to receive facility time.

For home practices please select the building and then in additional comments select specific rooms you are requesting.

For off-campus please just fill in the comment boxes all the information needed of facilities day and times. Just put something generic and the time and day fields listed. I just want to be able to track your off-campus info.

Practice Request Information

Please submit a different request for each facility you would like to request. For example, in the fall if you are looking to book outdoor field time and weight room time. Please submit a request for your outdoor field request, and then go in and fill out a separate request for your weight room request time. Same if you are requesting some outdoor time and indoor time. Do it separately please. Email me with any questions you have before submitting this.

Home Event Request






Please check if your off campus facility requires a Certificate of Insurance from GVSU to host. If it does, please upload a pdf or word document answering the following questions in the next question.

  1. Club name
  2. Date(s) of rental
  3. Location time
  4. Address of location
  5. Time(s) of event(s)
  6. Name(s) of other school(s) or university(s) participating
  7. Copy of rental contract
  8. GVSU contact person for event (organization president or coach)
