Practicum Site Supervisor Agreement

*This form should be completed by the Site Supervisor*

All practicums must have a full-time professional serving as a site supervisor. Site supervisors are expected to provide onboarding, training, regular contact, and necessary feedback for a meaningful practicum experience.

I agree to provide the following as a practicum supervisor:

• Onboarding, training, and necessary resources to complete required tasks/responsibilities of practicum position (including technology and workspace).
• Discussion around expectations (communication, attendance, office culture).
• A work schedule in collaboration with the student.
• Help with the development of practicum goals and in acquiring experiences that enhance designated areas for development by using the ACPA/NASPA competencies.
• Weekly check-in meetings with the practicum student to debrief experiences, provide feedback, and support student learning.
• Help in achieving at least 100 practicum hours.
• Complete the supervisor evaluation at the end of the practicum experience and share with the student and faculty advisor
