Fall 2026 Name Badge Order
MPH Students - It's time for the annual Student Name Badge Bulk Order!
Current and incoming MPH students have until noon, September 16 to order an official MPH student name badge. All students are encouraged to order at least one name tag, to wear while performing class-based work in the community (practicum, project, summer intensive, etc.), attending conferences or other professional training/events, or other instances when it's important to rep yourself as a GVSU MPH student! It is our goal to have the badges in time to hand out to students attending the Michigan Premier Public Health Conference in early October.
Price per badge is $9.00.
Deadline to order AND make payment is September 16 (12:00 p.m./noon)
Payment can be made with a credit card through the form, or you may indicate on the form that a check payment will be submitted to the Public Health office. Check payments delivered in person or via the mail must be received by our office no later than 12:00 p.m. on September 16.
The MPH bulk order happens once each year, so we encourage you to order your name tag(s) now. Badges ordered outside the bulk order window will cost significantly more.
