MPH Student Nametag Order

MPH students are encouraged to have an official student nametag, to wear during class-related work in the community (practicum, project, etc.), conferences, and other professional-related activities. Orders are typically only placed once per year, in order to take advantage of a bulk discount price. Nametags ordered outside of this window will be significantly more expensive.

Engraved tags will include both your first and last name, and will be engraved exactly how you enter them here -- enter the name you wish to display, and be sure to double-check your spelling.

Payment can be made using a credit card (through this form only!) or mailed/dropped off to Heather Koster (RFH 106 - checks should be made payable to "GVSU"). All payments must be received no later than noon on September 16, 2026.

The MPH Student nametag is "Grand Valley blue", 2x3 inches, with white engraving. First and Last names should be included, capitalizing only those letters that are normally capitalized (see the sample below). The tag features a magnetic back/fastener to make attaching it easier and to prevent damage to clothing.

Students are encouraged to have at least one nametag. Some students prefer to order additional tags for convenience, or in case one gets lost or damaged. Ordering a replacement tag outside of the department bulk order will result in a much higher price point.
