ATCAS GVSU Additional Forms

Both forms listed below are to be downloaded, scanned, and uploaded to ATCAS under documents.


GVSU Hour Verification Form

  • Please complete one form per observation site. If you attend one site more than 10 times please fill out another sheet. 
  • The top two sections are to be typed (keeping formatting), with no abbreviations.
  • You must use this forms, no other documentation will be accepted.
  • The forms must be fully completed.
  • Supervising Athletic Trainer
    • Must be certified and if applicable, based on state regulations, either licensed/registered by that state. 
    • The individual may have multiple credentials. However, athletic training credential must be in good standing and not expired.
    • The athletic trainer must provide a signature for each date/time completed.

 

  • Hours can be obtained at high schools, colleges, clinics, or any locations where a licensed athletic trainer is employed.  Intercollegiate Athletics through the GVSU Athletic Training Department is reserved for ATH 210 Students ONLY. If you are not enrolled in ATH 210 course, in the current semester, please DO NOT contact the Athletic Department personnel.
  • In accordance with GVSU Student Code of Conduct 2.1.3 - students shall not provide false information. Falsification of any information on this sheet will result in the candidate being omitted from the pool of applicants.

Technical Standards

The Master of Athletic Training (MAT) at Grand Valley State University (GVSU) is a rigorous program that places specific requirements and demands on admitted athletic training students. Students must demonstrate specific intellectual, technical, physical and behavioral essential skills set forth by the athletic training profession. The objective of this program is to prepare graduates, to enter a variety of employment settings, and to render care to a diverse patient population engaged in physical activity.
