Secondary Transition Community of Practice (CoP)

Purpose:

To build capacity across the state in Secondary Transition, with a focus in 5 key areas including 1) Education, Transition Services, and the IEP, (2) Planning for Competitive Integrated Employment, (3) Empowering Students, (4) Independent Living, and (5) Interagency Coordination and Collaboration.

Criteria for Participation:

  • Open invitation (each RCN is encouraged to have representation).
  • Currently in a role that involves directly supporting secondary transition planning and programming from ages 14 to 26.
  • Willingness to actively contribute to the CoP priorities through participation in meetings and sharing ideas, resources, materials, and practices that align with best practices.

Current Priorities:

  • Enhance local implementation of Secondary Transition programming through sharing and disseminating resources and tools that align with the 5 Key Components of Secondary Transition.
  • Highlight information covered through the Online Self-Paced Learning.
  • Engage in community problem-solving to address challenges faced by CoP members and the regions they represent.
  • Cultivate leadership and empower existing leaders in the area of Secondary Transition.
