Student Family Member and Dependent Membership Form

Please Note:

The student(s) sponsoring dependents must be enrolled at GVSU for the semester during which their dependents are purchasing a membership. The enrolled student(s) do not need to purchase a membership or membership card for themselves.
Student Family/Dependent Membership cards are $25/person/semester.
Only one semester can be purchased at a time; membership rates are not eligible to be prorated.
Parking permits are required for parking on the GVSU campus. A permit can be obtained at the GVSU Public Safety Office with a current membership card and driver's license. Parking permits are for Rec Center use only; improper use of permits may result in campus Parking/Traffic fines.
Participants under 14 years of age:
- Are permitted to utilize the Cardiovascular Fitness Areas and Rec Center Courts only and must be accompanied by an adult at all times.
- Are prohibited from using any Selectorized or Weight Lifting equipment in all areas of the facility, regardless of adult supervisio
