GVSU Music Department agrees to loan a musical instrument to the person listed above, referred to as 'User' herein:

The loan shall be for the length of this signed agreement as noted above (*due date). User agrees to pick up instrument from the Music Department and return instrument to same at the end of the rental term. User understands and accepts that this is a loan and that there is no fee charged for the use of this instrument if it is returned and is in good condition upon return, normal wear and tear excepted. Deposits are not required for musical instruments loaned pursuant to this agreement. However, should a loaned instrument be stolen, the user will be charged the replacement cost of the instrument. If an instrument loaned pursuant to this agreement is not returned at the end of the loan period, the replacement cost of the instrument will be assessed to the User.

The User assumes full responsibility for the instrument and agrees to take good care of it. User agrees to notify the Music Department if the instrument (or a part) is stolen, damaged, or in need of adjustments or repair and to consult with the Music Department if repairs are required.

It is expressly agreed that the use of any musical instrument from GVSU is at the User's sole risk and User assumes any and all risk from the use and/or operation of a musical instrument from GVSU. It is further expressly agreed that GVSU staff, advisory committee and officers, representatives, employees, or students, shall not be liable for any claims, demands, injuries, damages, actions or causes for action whatsoever to the User or any other person arising out of or connected with the use and/or operation of any musical instrument from GVSU. The User hereby expressly forever releases Grand Valley State University from any and all claims, demands, injuries, or actions.
