Required for All New Students
All new international students are required to complete 5 forms that must be submitted before the first week of class. All 5 forms are required for immigration reporting and to create Social Security Application packets.
#1 | Immigration Documents
In order to report your arrival to USCIS, we must collect your immigration documents.
#2 | Personal Information
In order to maintain your immigration record, GVSU must collect your personal information via this form:
Tips for completing this form:
- If you are living on campus, you can look up your address here.
- If you do not yet have an address or do not know it, please use the Padnos International Center address:
Address: 1 Campus Drive
Post Code: 49401
- Until you have an American home or cell/mobile number, please use the office number for both: (616 )331-3898
- Port of Entry is the first city through which you entered the US.
For example, if you are flying Beirut to Paris, Paris to Chicago, Chicago to Grand Rapids, Chicago (CHI) would be your Port of Entry because it is the first city you came to when entering the US.
#3 | Immigration Regulations
Please take a few moments to complete ONE of the following mandatory forms.
For students with an I-20
For students with a DS-2019
# 4 | GDPR
In order to be compliant with GVSU's policy, all new students must complete this form for European Union General Data Protection Regulation.
#5 | Health Insurance Reporting
J-1 visa holders are required to have health insurance. F-1 students are strongly encouraged to purchase a health insurance policy. Health care in the USA is extremely expensive.
GVSU collects this information to help support students' health care needs.
