J-1 Short-Term Scholar Application

Please submit this form if you are applying to be a short-term scholar at GVSU. The information you provide will be used to create a DS-2019 document for your J-1 Visa Application.

You will be asked to upload a scan of your Passport, Department Invitation Letter, Resume/CV, Proof of English Proficiency, and Proof of Funding. Additionally, any documents related to previous immigration statuses and dependents will be required, if needed.

All information must be complete at time of submission. You cannot revisit this form to make changes. A copy of your responses will be emailed to you after submitting this form.

Proof of Funding

In order to issue a DS-2019 document for a J-1 Scholar, GVSU is required by Federal Law to collect evidence that the J-1 applicant has adequate funding to cover their program and expenses at GVSU. We calculate that as: $1000 month for rent + $100/week for food and general expenses + any additional program-specific expenses such as Tuition.

Prior to submitting this application, you should have been in communication with a sponsoring department at GVSU as well as the Scholar Services Team in the Padnos International Center who will assist you in calculating the exact amount needed.

If your hosting department is providing funding that covers the entire amount needed for Proof of Funding, please upload the Host Department Invitation Letter again here. If the funding in the Invitation Letter is not sufficient, please upload a personal bank statement that shows at least the remaining amount needed for your Proof of Funding.
