NOTICE OF PATIENT PROTECTION

You do not need prior authorization from the Health Benefit Plan for Grand Valley State University or from any person (including a primary care provider) in order to obtain access to obstetrical or gynecological care from a health care professional in our network who specializes in obstetrics or gynecology. However, the health care professional may be required to comply with certain procedures, including obtaining authorization for certain services, following a pre-approved treatment plan, or following certain procedures for making referrals. For a list of participating health care professionals who specialize in obstetrics or gynecology, contact your HR Business Partner.

WOMEN'S HEALTH AND CANCER RIGHTS ACT OF 1998

Women’s Health And Cancer Rights Act Of 1998  (Also Known As “Janet’s Law”)
Did you know that your health plan, as required by the Women’s Health and Cancer Rights Act of 1998, provides benefits for mastectomy-related services? These services include reconstruction and surgery to achieve symmetry between the breasts, prostheses, and treatment of complications resulting from a mastectomy (including lymphedema). Call Priority Health at (800) 956-1954 for more information.

Appropriate treatments will be determined by consultation between the attending physician and the patient. Plan coverage is subject to the annual deductibles and co-insurance provisions that apply to other similar medical treatments.
