  • September 15 for winter semester admission and field placement

for Teacher Assisting and Student Teaching

Applications are available as downloadable PDFs and are submitted in-person at the College of Education Student Information and Services Center on the downtown Pew campus, 401 C Richard M. DeVos Center. Packets must be completed at the time of admission. In addition to the application, an official copy of the reading, mathematics, and writing sub-scores from the ACT or SAT must be on-file with the Office of Admissions at GVSU. Students who postpone admissions must reapply. Students should apply during the semester before they expect to do their first field placement.

Please select the appropriate level application below. Save the .pdf form to your desktop and fill the form out ONLINE, typing in the required data - this .pdf is a writeable form. Print the entire completed application inserts/forms and turn all of the materials in during the application window outlined above.

Teacher Assisting Applications:

The following documents are needed when turning in your Teacher Assisting application to the College of Education:

Secondary Teacher Assisting Application

  • Students seeking a major in Music Education must apply during the winter semester (February 1) for fall admission (i.e. fall Teacher Assisting).

Special Education Teacher Assisting Application

  • Students seeking Elementary certification with Special Education endorsements must apply during the winter semester (February 1) for fall admission.

Student Teaching Applications

The following documents are needed when turning in your Teacher Assisting application to the College of Education:

  • Up-to-date Résumé (unstapled on plain white paper). Include three references, current e-mail address, and Teacher Assisting experience (and Student Teaching if Special Education). Please don’t use color or shading on your résumé as it is scanned into a PDF file and emailed to Principals and/or Cooperating Teachers for potential placement.

  • Students seeking a major in French, German, or Spanish must pass the Oral Proficiency Interview (OPI) with Advance Low or better prior to their Student Teaching semester.

The COE Advisor Recommendation form will be provided by the COE advisor when students meet with them for their advising appointment. The form is not available online. The following links provide access to the Major Advisor Recommendation forms.

Elementary Major Advisor Recommendation Form

Note that this form is also used for advising students pursuing the Elementary ESL and Spanish minors within the Elementary CSAT program. At the time of application, the completed form is to be submitted with the application packet to the College of Education Student Information and Services Center.

CSAT (Special Education) Major Advisor Recommendation Form

Positive Recommendation Form

Recommendation Form- General Education

  • Submit one from an unrelated individual who can address your ability as a prospective teacher.

Recommendation Form- Special Education

  • Submit one from an unrelated individual who can address your ability as a prospective teacher in Special Education.

Verification of Service (25 Hours) Form

Verification of Service Form- General Education

  • Documents 25 hours of experience with children or youth.

Verification of Service Form- Special Education

  • Documents 25 hours of experience working with children or youth with special needs, e.g., camp experience, Special Olympics, respite care.
