CHM 125/126 to CHM 100 Parachute Form

Please fill out this form if you would like to transfer your registration from CHM 125/126 to CHM 100 (sometimes referred to as the "parachute" option).

This form must be submitted by 3 p.m. on Friday, Sep. 25.

Note: You are only eligible to pursue this option if you have been contacted by your CHM 125 Instructor. Please contact that instructor if you have any questions.

By submitting this form, you understand that:

You are eligible for this option only if you have been contacted by your CHM 125 lecture instructor.
You will no longer be registered for CHM 125 lecture and CHM 126 lab. You will receive a 100% refund for the CHM 125/126 course and are expected to pay for the CHM 100 course at the time of the switch. This should resolve itself by applying any refund from CHM 125/126 to CHM 100.
This will reduce your total number of credits by 1, as CHM 125/126 is 4 credit together and CHM 100 is a 3 credit course. It is your responsibility to confirm that this will not impact your enrollment status and financial aid eligibility
There will be no late registration fee.
At the end of the semester, you will receive a grade for CHM 100 only; your transcript will not indicate that you made this switch.
This action is irreversible.
