Laboratory repeat waiver form (CHM 231)

This form must be submitted by 5 pm on the Friday of the first week of classes.

This form is for students who are repeating a chemistry class and wish to opt out of the lab portion of the class the second time around. There are three important considerations:

  • This is only for CHM 231. If you wish to get a lab waiver for CHM 109 or 234, please return to the menu on the CHM website and choose the correct form. For all other CHM classes (ie. 115, 116, 241, 242. etc), no waivers are permitted--you must repeat the lab if you are repeating the lecture.
  • In order to qualify for a repeat lab waiver, you must have scored 90% or higher in lab portion of the course the first time you took it.
  • Important note: if you waive the lab the second time you take the class, your semester grade will be calculated using only the grades from the lecture portion of the course. The lab score often pulls the grade up, so consider the impact of this policy before you decide to waive the lab.

Information about the class when you first took it

Information about the class that you are taking this semester

After you submit this form, we will confirm that your previous lab was 90% or higher. We will then email you, your lecture instructor and your lab instructor to verify that:

  1. you will not be attending lab this semester.
  2. your course grade at the end of the semester will be calculated without any lab grade.

Important! You are not approved for the waiver until you have received an email receipt from us stating that you are approved. You should attend all lab sessions until you have been approved. If you do not hear back in 2-3 days, feel free to contact Prof. Dave Leonard at [email protected] or 616-331-3241.
