By checking each box I acknowledge that I am aware of the following information: * A concussion is a traumatic injury, which student-athletes should report to the medical staff. A concussion can affect the student-athlete's ability to perform everyday activities, and affect reaction time, balance, sleep and classroom performance. You cannot see a concussion. Some symptoms may be noticed right away and others may take days. Student-athletes shall not return to play in a game or practice on the same day that they are suspected of having a concussion. If I suspect a student-athlete has a concussion, it is my responsibility to have that student-athlete see the medical staff. I acknowledge that the GVSU Medical Staff (Team Physicians and Athletic Trainers) have Independent Medical Care authority. Following a concussion, the brain needs time to heal. Concussed student-athletes are much more likely to have a repeat concussion if they return to play before their symptoms resolve. I am aware that baseline measurements are taken on every student-athlete who competes on a varsity athletic team. I am aware that student-athletes diagnosed with a concussion will be assessed by the medical staff. They must be asymptomatic for at least 24 hours before being directed by an AT to begin a graduated exercise progression with a step wise process. I understand the importance of limiting head trauma exposure in a manner consistent with the NCAA Recommendations: Preventing Catastrophic Injuries and Death in Collegiate Athletes. I understand the return to learn procedure and that any student-athlete sustaining a concussion will not attend classes on the day of the concussion. Student-athletes will work with campus DSR for classroom accommodations. I understand a student-athlete must fully return to the classroom without accommodations before they will be fully released for athletic participation.
