Nicotine eCheckup
The Nicotine eCheckup To Go is an online assessment that will ask you several questions about your relationship with nicotine. The purpose of this assessment is to give you information to help you live the healthiest life you can. If you have questions about why a specific question is being asked, you will have the option to click a link and learn more about why this question is being asked of you.
There is not a “one-size-fits-all” approach to nicotine use, nor is it our goal to force anyone to make any changes that they are not ready for. Our hope is that you are safe, smart, and responsible in your choices around substance use. Once you have finished the assessment, you will be provided individualized feedback and information compared to that of your GVSU peers. If you would like to return to this information at any time, simply type in the code that you are given during the assessment. Alternatively, you will have the option to retake the assessment if your use changes.
Please take the eCheckup as honestly as you can. We will not ask for your name and will not collect any identifiable information. This data will be kept anonymous. Please think carefully about each question and be as accurate as possible in your responses so that the feedback you receive will be tailored specifically to your needs.
To verify the completion of this assessment, please print the certificate at the end of the assessment to provide as proof of completion to the referring party.

Please be sure to adhere to the Student Code, Housing and Residence Life Community Living Standards and local, state and federal law.
Medical Amnesty provides some exemption to reduce barriers for minors to seek medical attention in the matter of an alcohol or drug emergency.
