GVSU Health Compliance COVID-19 Vaccine Exemption Requests

The following GVSU faculty, staff and students must have documentation of being 'up to date' for COVID-19 vaccines, prior to starting their placement or engaging with staff from a clinical placement location. 

  • Faculty, staff and students in health/health related programs enrolled in health compliance (CastleBranch) for the current academic year.
  • Students, regardless of their major who. during the current academic year, will have a course, fieldwork, internship or other experiential learning placement at a health care organization.

Individuals with a recognized medical condition for which the COVID-19 vaccine is contraindicated, or a sincerely held personal religious belief that prohibits them from receiving the COVID-19 vaccine, may submit an exemption request to the Health Compliance COVID Vaccine Exemption Committee. 

University exemptions (medical or religious) may not be extended for faculty/staff/students enrolled in health/health related programs OR who are in a program where they will participate in experiential learning at, or interact with staff from, health care locations. A new request must be submitted for review by the Health Compliance Vaccine Exemption Committee.

MEDICAL EXEMPTIONS - REQUIRED FORM

The Health Compliance COVID-19 Vaccine Medical Exemption Request form is required for individuals who are requesting an exemption due to a medical condition for which all currently available COVID-19 vaccines are contraindicated based on documentation from the Centers for Disease Control and Prevention (CDC).

The form's 2nd page must be completed by a health care provider with knowledge of your medical condition, and both pages must be submitted to Med+Proctor at the same time (see Med+Proctor submission information below).

RELIGIOUS EXEMPTIONS - REQUIRED FORM

The Health Compliance COVID-19 Vaccine Religious Exemption Request form is required for individuals who are requesting an exemption due to a sincerely held religious belief, practice or observance that prohibits them from receiving the COVID-19 vaccine.

The completed form must be uploaded to Med+Proctor (see Med+Proctor submission information below). 

Review Process

All health compliance COVID-19 vaccine exemption requests will be reviewed by the Health Compliance COVID Vaccine Exemption Committee within 10 business days after submission of required documentation (as volume allows).

A written response will be sent by the committee and may include:

  • Approval for an exemption to the vaccine requirement for a period of no more than the academic year of the approval. 
  • Request for additional information.
  • Denial of the request for an exemption to the vaccine requirement. If the exemption request is denied, the reason will be provided, as well as information to appeal the committee’s decision

Individuals approved for an exemption will be required to follow the protocols for unvaccinated individuals set by the university and the placement site.

NOTE: The university and Health Compliance COVID Vaccine Exemption Committee reserve the right to request an earlier review based upon the availability of new vaccines and/or new clinical information from the CDC.

Health Compliance COVID-19 Vaccine Exemption FAQ

  • Download the appropriate exemption request form. 
  • Submit both pages of the completed form to Med+Proctor
  • Religious exemptions require a personal statement explaining your sincerely held religious belief, practice or observance, which also addresses: how receiving the COVID vaccine or booster would violate the belief, how the belief is practiced in your daily life, how the belief extends to other products that are similar to the COVID vaccine and, if applicable, whether this is consistent with the official guidance and direction of your religious/spiritual tradition. 
  • Directions and the link to submit your completed form to Med+Proctor can be found at the bottom of this page. 
  • You will receive a written response from the Health Compliance Vaccine Exemption Committee within 10 business days of submitting the completed exemption request form on Med+Proctor.
  • You will receive a written response from the Health Compliance Vaccine Exemption Committee explaining the reason for the denial, and providing the steps for appeal.  You may not be on site or have other interaction with placement site staff or patients until the exemption is approved, or you have completed the primary vaccine series.

MED+ PROCTOR SUBMISSION INFORMATION

  • Submit your Health Compliance COVID-19 Vaccine Exemption Request form to Med+ Proctor by selecting the "Upload Health Compliance COVID-19 Vaccine Exemption Request Form" link below, Once on the Med+Proctor site, select "Supporting Documents" under Document Type. 
  • *Uploading your supporting documents to Med+Proctor is FREE.  
    • The $10 fee is for individuals who desire to use Med+ Proctor's enhanced service**. If you don't want the enhanced service, be sure to click on "To continue without paying." It is written in smaller type under the $10 fee info. Click on that to continue without paying. For any other questions or problems using the service, Med+Proctor has an online chat to help. 
  • **What is the enhanced service?  The $10 optional upgrade fee is for a Med+ Proctor Pro Account. The MP Pro Account allows you to have lifetime access to your MP account. You will be able to view all of your uploads and you will be able to download your uploads as well. This allows you to have quick and easy access to your immunization records in the future. If you don't want those perks you can select the gray "no thank you" button to upload with a general account.  
  • ***There is another optional fee to expedite document review. This is NOT recommended for GVSU students or staff as GVSU staff currently review all exemption documentation and paying this fee will not impact the timing of the review.
