Employee Accommodations
If you are a GVSU employee seeking accommodations for a temporary or permanent disability that will aid you in performing your essential job function, please complete the confidential accommodation request form below. Documentation of the qualifying disability should be from the medical practitioner who diagnosed the disability or who is providing care related to the disability. Every effort will be made to keep all communications confidential in reviewing requests for accommodations.
