Things to consider:

Compare your new spouse’s benefits with your benefits so that you can make an informed decision on whose coverage will best suit your situation

Notify the Benefits Office within 30 days of the date of marriage

Enroll spouse/dependents into GVSU Benefit Plans
-
Complete Medical Plan Enrollment form
-
Return form to HR within 30 days of the life event.

Bring in marriage license with raised seal.

Drop medical, dental, vision coverage
-
Notify Benefits Office in writing within 30 days of the marriage

Bring or send in a copy of the final Divorce Decree or Notice of Legal Separation

Increase Flex Spending Account
- Complete FSA Enrollment Form
- Return form to HR within 30 days of the Birth

Change your name, marital status, and add/change life insurance beneficiaries.

Increase or enroll your spouse and/or dependents in the supplemental life insurance program.
