College Prep Summer Simulator Camp 2024

Welcome Camper! We are excited to have you with us July 14 - 19 at Grand Valley State University. Our College Prep Summer Simulator is an opportunity for you to learn about what it is like to be a college student while also enjoying fun activities around the Grand Rapids area! To make sure we give you the best experience possible, please read through and answer this form carefully. We will be using it to gather information about you so that we can ensure you can participate in all the fun while also making sure you are safe and have access to everything you may need while you are with us! Please make sure that the email you provide is the email that you check the most! We will be sending out super important information for your camp experience and we don't want you to miss out on any of it!

Acknowledgement of Confirmation of Attendance

Completing this form will serve as a confirmation that you will be attending the College Prep Summer Simulator Program from July 14 to July 19. This form serves as your placeholder in our program, as we do have a maximum amount of students we can admit. If at any point you would like to retract your confirmation and your spot in the program, please contact Shelby Powell at [email protected] so that we can ensure another student has the opportunity to attend.

If you are a student from/near the Detroit area, will you be riding the bus? The bus will pick up and drop off at University Preparatory Academy - 600 Antoinette St, Detroit, MI 48202. Times will be shared to the students who sign up for transportation. *



Health and Wellbeing

It is important that we know of any allergies or restrictions you have so that we can be sure to keep you safe and accommodate any needs. We also will be collecting medications at student check-in as not only a safety measure to ensure that you and your medications stay safe, but also to ensure that you have access to your medications whether we are on or off Grand Valley's campus. Our camp nurse will be distributing the medications at the time you need them each day of camp! The medications will all be labeled with your name, the medication name, when and how often you take it, and the amount you need at each dose. We will also have over-the-counter medications such as Tylenol, Motrin, Cough Medicine, etc. available to any student that needs it in our camp supplies.

During camp we will be participated in a number of fun activities! GVSU as well as the locations these activities will be hosted require liability forms to be completed. Roommate Pairing

While you are at camp, you will be sharing a room with a roommate! To make that process a little easier, we want to know more about you so that we can try and find you a perfect match for the week!
