Everyone is required to complete a self-assessment form before being admitted into the Fieldhouse Arena. We encourage all attendees to complete the self-assessment form in advance of arriving on campus.

To expedite the check-in process we recommend you take screen shots of your green checkmark if more than one attendee is using the same mobile device to complete the self-assessment.
