I-9 Form
Instructions For Completing the I-9 Form
You must have the correct original documents to show to the staff person completing the form. Check back of I-9 form for acceptable documents.
1. All student employees, upon being hired, must complete Section 1 of the I-9 Form. They must print or type their complete name, address, and date of birth.The block must be checked that correctly indicates the student's citizenship or immigration status. Most students will check block one. However, if the second block is checked, the student's Alien Registration Number must be provided. If the third block is checked, the student's Alien Registration Number or Admission Number must be provided, as well as the date of expiration of that status, if it expires. (If you are hiring a foreign student the International Center will help with I-9 Forms ext. 1-3898.) All students must sign and date the form.
2. Employers (PERMANENT GRAND VALLEY STAFF) must complete section 2 by examining evidence of identity and employment eligibility:
a. You will find a list of acceptable documents box for List A or List B and C on the back of the I-9 form.
b. Record the type of document used and any applicable identifying numbers under the appropriate column.
c. Sign the certification section. This signature certifies that the signee has examined the required documents. A student or temporary employee cannot certify the I-9 Form.
3. The original completed I-9 Form must be forwarded to the Student Employment Office.
4. The I-9 must be completed within three calendar days of the starting work.
Note: Individual departments should not keep copies of the completed I-9 form. The original must be on file in the Student Employment Office.
