A blank prerequisite override request form may be downloaded below (Word document).  Read carefully and follow all of the instructions on the form. Submit your completed form with all required documentation attached to the Physics Department office (PAD 118), either in person, via email (phyoffc@gvsu.edu), or FAX (331-3740). Missing or incomplete attachments will delay your request.
