    UMR is our, TPA, that administers the Standard PPO and High Deductible Health Plan PPO Plans and Flex accounts.  In addition to processing claims, UMR provides you around-the-clock, online access to a wealth of information to help you manage your benefits and your health. You can check your claims, find a provider, review eligibility, download forms, request an id, check your flex account balance and learn more about your health benefit plans.  
    • Cofinity (PPOM) - (Physician Network for theStandard PPO and High Deductible Health Plan PPO Plans)
      Find out about PPOM, Look at the Provider Directory, See Provider Services and much more.
    • PHCS - If you are enrolled in the GVSU High Deductible PPO or Standard PPO Plans and are travelling outside of the Cofinity network, (Michigan, Indiana and Ohio), look for providers in the PHCS network. Claims for providers participating in the PHCS network will be processed as in-network. You can find the contact information for PHCS on the back of your medical plan ID card.  800.678.7427
