TRIO Talent Search Program Application (2023-2024)

First priority for admittance will be given to students in low-income households whose parents or guardians have not completed a 4-year college degree, and who currently attend one of our target middle schools or high schools. Other applicants may be considered if spaces are still available.

There is no time limit to complete this form, but your progress will not be saved if you leave this page partway through. 

In order to complete it, you will need the following information:

  • Phone numbers for 2 emergency contacts other than a parent/guardian
  • The student's grade point average (GPA) or most recent grade report
  • Your household's taxable income from your most recent tax forms

Consent and Academic Record Release

Please read each statement carefully. Your signature implies consent and agreement.

  • We certify that all information provided on the application is the truth and correctly noted to the best of our knowledge.
     
  • We authorize the release of the applicant’s school records (transcripts, standardized test scores, free/reduced lunch record, IEP), and college enrollment/National Student Clearinghouse information (financial aid, admissions information, postsecondary degree attained) to TRIO ETS, and authorize TRIO ETS to report on student outcomes and demographics to the U.S. Department of Education.
     
  • We understand that participation with TRIO ETS does not guarantee admission to college or receiving financial aid/scholarships.
     
  • We understand that students are expected to participate in program workshops or meetings with an advisor twice a semester (at school or virtually), make adequate academic progress, communicate with the TRIO advisor during the academic year, submit required documents, and follow the student code of conduct.  I, the student, agree to meet these expectations in order to maintain my membership in the program.
     
  • We understand that all information on this application will be confidential per FERPA federal law, and that information will be shared with the U.S. Department of Education for annual reporting purposes per grant guidelines.
     
  • We authorize the use of the student's image or work in TRIO ETS publications, media, and social media releases.  (Call our office if you have special circumstances and do not want your child's image used for anything other than his/her file.  We can make exceptions.)
     
  • I, the parent or guardian of the above student, agree to communicate with TRIO ETS staff; agree to encourage my son/daughter's participation in TRIO ETS meetings, activities, and workshops; and agree to encourage him/her to attend college or a post-secondary certificate program the fall after high school graduation.
     
  • I, the parent or guardian of the above student, give consent for him/her to participate in virtual TRIO ETS workshops/events, and field trips, including those sponsored by TRIO ETS but presented by other institutions, businesses, organizations, and persons.

Other Recommendations for Consideration

Please Note:

Completing this application does not guarantee acceptance into the program. If your application is approved, we will send you a welcome letter via mail or email to let you know your application has been approved. 

You are welcome to reach out to our office if you have any questions about the status of your application or how to get in touch with a TRIO Advisor.

Expectations for students accepted into the program: 

  • Students must attend workshops (either virtual or in person) each semester in order to remain in the program. 
  • Students must communicate regularly with their TRIO Advisor and respond to communication from program staff.
  • Students must maintain a 2.0 GPA. If in danger of falling below a 2.0, contact your TRIO Advisor to construct an academic plan.
  • Membership in the program continues until graduation from high school, unless you notify us that you wish to leave the program sooner. 

We also encourage you to attend our field trips, virtual activities, and summer programming.

When you reach your senior year, we may ask for your Social Security Number in order to verify your college enrollment through National Clearinghouse.
