Provider Survey

The START (STatewide Autism Resources & Training) Project is looking to expand upon the information that is currently offered to parents and family members of people with Autism Spectrum Disorders.  We would like to provide a listing of a wide range of local service providers such as doctors, dentists, hair salons, respite programs, and supplemental therapies (e.g. music therapy, equestrian therapy) on our new online resource center website.

In order to develop such a list we need your help! If you know service providers in your area that offer services or that caters to the needs of people with ASD, please provide their information so that we can share the information with others in your area.  There are several categories of providers listed below – please provide information about the provider(s) such as name, address, phone, email, and website.  If there is a category that you feel should be included please provide the category and provider information in the “Other” space at the bottom.

In our service provider database there will be an option for users to review the providers and provide feedback, similar to systems used by Amazon.com.
