Independent Study Form [PDF]

There are 3 steps you must complete before you can register for an independent study (399) section.

1.       You must find a faculty member to work with. It can be any professor who has expertise in the subject you want to study (It does not have to be the faculty member whose name is listed in Banner by all the 399s—that is just for administrative purposes).

2.       Print off this form or pick one up from the School of Communications Office (290 LSH). You and the faculty member you will be working with must fill out the form. This form gets turned back into the School of Communications Office.

3.       Once you turn in the Independent Study form, you can then go online into Banner and request a permit for section you wish to enroll (please take note what that section number is when you are working with your faculty member).

Once those 3 steps have been completed, a permit can be entered for you and you will then be able to register for your section of 399.

COM 498 Senior Thesis/Project Form [PDF]

There are 3 steps you must complete before you can register for a COM 498 Senior Thesis section.

1.       You must find a faculty member to work with. It can be any professor who has expertise in the subject you want to study (It does not have to be the faculty member whose name is listed in Banner by all the 498s—that is just for administrative purposes).

2.       Print off this form or pick one up from the School of Communications Office (290 LSH). You and the faculty member you will be working with must fill out the form. This form gets turned back into the School of Communications Office.

3.       Once you turn in the Senior Thesis form, you can then go online into Banner and request a permit for section you wish to enroll (please take note what that section number is when you are working with your faculty member).

Once those 3 steps have been completed, a permit can be entered for you and you will then be able to register for your section of 498.

Degree Cognate Substitution Request Form (CLAS) [PDF]
Print and complete this form for a degree cognate substitution request. Bring to the SoC office in 290 LSH once completed.
