Written plans for the proposed flexible work arrangement should be submitted to your immediate supervisor and will require approval and signatures from both your immediate supervisor and appointing officer. This will help to facilitate understanding between all involved parties. Communication is critical to the success of a FWA. The written plan document will also provide an essential key to refer back to during the FWA.

For all staff, the application form for a FWA should be submitted to Human Resources, 1090 JHZ. This form will be filed with the Human Resources Specialist as a tool to track FWA usage. For hourly staff, the Human Resources Specialist will notify payroll of the FWA, by email (
