Med+ Proctor Overview

*Uploading your proof of vaccination to Med+Proctor is FREE.  The $10 fee is for individuals who desire to use Med+ Proctor's enhanced service**. If you don't want the enhanced service, be sure to click on "To continue without paying." It is written in smaller type under the $10 fee info. Click on that to continue without paying. For any other questions or problems using the service, Med+Proctor has an online chat to help. 

**What is the enhanced service?  The $10 optional upgrade fee is for a med proctor pro account. The MP Pro Account allows you to have lifetime access to your MP account after graduation, you will be able to view all of your uploads and you will be able to download your uploads as well. This allows you to have quick and easy access to your immunization records in the future. if you don't want those perks you can just click the gray no thank you button to upload with a general account.  

***There is also another optional fee for expediting of documents. At this time, this is NOT recommended for GVSU students or staff as GVSU staff are currently reviewing documents submitted as it relates to exemption requests and paying this fee will not affect the timing of the review of exemption requests that have been submitted. 

*Supporting documents for exemption requests should be submitted to Med+ Proctor by selecting upload a document and then select Supporting Documents under Document Type.

You can upload proof of a vaccination to Med+Proctor, an easy and secure what to upload your COVID-19 vaccination proof.

There is no cost to use Med+ Proctor.

The information you provide is confidential and will only be used in association with GVSU vaccination requirements. As a cloud hosted program, Med+Proctor does not have access to individual data, as detailed in the Med+Proctor privacy policy .

Yes you are required to register an account and submit records via Med+Proctor. This ensures your immunization records are safely and securely transferred to GVSU. 

GVSU COVID-19 Vaccine Exemption and Postponement Requests

GVSU will recognize medical exemptions, religious exemptions, and temporary postponements to the requirement to be vaccinated for COVID-19.   All exemption and postponement requests will be reviewed by the Vaccine Exemption Review Committee and a written decision provided within 5-7 business days (as volume allows).  At this time, the university’s decision on exemption/postponement requests will also apply for individuals enrolled in health/health related programs assigned to a health care agency that requires vaccines.

Medical Exemptions

The medical exemption form is for individuals who have a medical condition that prevents them from receiving the COVID vaccine prior to the timeframes outlined by the university or a health care agency where the individual may be scheduled for experiential learning. As you fill out the form, you will be asked to explain the reason for your medical exemption request.  You must provide additional documentation such as the Provider Documentation Form when requesting a medical exemption.  Please upload your supporting documentation to Med+ Proctor.

Religious Exemptions

The religious exemption form is for individuals who have a sincerely held religious belief that prevents them from receiving the COVID-19 vaccine. When completing the form you will be asked to explain why you are requesting a religious exemption. You will need to include a statement explaining your sincerely held religious belief that supports not getting the COVID-19 vaccine.  The exemption committee may ask for additional documentation based on the information you provide.

Temporary Postponement

The temporary postponement form is for individuals who are requesting to defer the vaccine requirement to a later date. When requesting a temporary postponement, you will be asked to explain why you are making the request and what your plan is for receiving a COVID-19 vaccine at a later date.  Please note: students can apply for a temporary postponement for the Fall semester vaccine requirement if their course load is fully remote and none of their required or elected activities will bring them to campus.

REVIEW PROCESS

All exemption requests will be reviewed by the Vaccine Exemption Review Committee and a written decision provided within 5-7 business days (as volume allows).

The response may include:

  • Request for additional information.
  • Approval for a deferral/postponement of the vaccine requirement for a specified timeframe.
  • Approval for an exemption to the vaccine requirement for a period of no more than one year from the date of the approval.  The university reserves the right to request an earlier review based upon new clinical information from the CDC or other medical and public health entities.
  • Denial of the request for an exemption to the vaccine requirement. If the exemption request is denied, the reason will be provided, as well as information to appeal the committee’s decision.
  • Individuals approved for the deferral or exemption from the vaccine requirement will be required to follow all university protocols for unvaccinated individuals, which may include participating in weekly testing.  Individuals with an approved exemption recognized by a health care agency where they are placed for experiential learning, will also be required to follow the health care agencies protocols.

If the exemption request is denied, the reason will be provided, as well as information to appeal the committee’s decision.

Individuals requesting an exemption due to medical reasons must provide an explanation of the medical conditions that would make it unsafe to receive the COVID-19 vaccine.  One example is a documented history of a severe allergic reaction to any component of the vaccine.

Individuals should explain why their “sincerely held religious belief” prevents them from getting a COVID-19 vaccination. Individuals requesting this exemption should  consider including supporting documentation that is relevant like a written statement or written information provided by a religious/spiritual leader.
