All of your health information is stored in one electronic file, so whether you visit the Metro Health GVSU Campus Health Center, a physician at our Allendale office, or the emergency room at Metro Health Hospital, all providers will be looking at the same information

To request copies of your health information be sent to other parties, you must complete a Release of Protected Health Information form.

Authorization to Discuss Medical Information

To disclose your health information with your parents, family members or legal guardians, please complete an Authorization to Discuss Medical Information form.
