Please review the following information carefully to learn about the requirements, obligations and limitations of this plan before submitting your e-request for membership. It is recommended that you print a copy of this page for your records prior to submitting.

I authorize GVSU to charge my account $160 - due immediately - for membership to the David D. Hunting YMCA for the semester indicated above to be paid to Grand Valley State University in accordance with the University's payment policies.

Joining Deadline: 11 pm on Friday, May 31, 2013

This membership provides access to participating YMCA of Greater Grand Rapids branches and is valid for one semester; therefore, you must process a new on-line request every semester to maintain active membership. Participating locations include David D. Hunting (Grand Rapids), Visser Family (Grandville), Wolverine Worldwide (Belmont), Southeast (Cascade), Lowell, Ionia, and the new Spartan Stores YMCA (Wyoming).

GVSU students are expressly prohibited from parking in the David D. Hunting YMCA parking areas at any time and are subject to towing at the owner's expense.

Membership Payment Options through GVSU:
Cash, check, money order, or e-check (no additional fee)
Credit or Debit Card ($15 fee) - only available for on-line payments.

Activation is automatic and is effective the first day of the semester - Monday, May 6, 2013. To begin using your student membership, visit the David D. Hunting YMCA to complete their forms and to obtain your ID card. Be advised that your membership will automatically activate regardless of whether you visit the YMCA.

Print a copy before submitting as you will not receive any other confirmation. Bring this form to the YMCA where you will complete your membership application, participation agreement and obtain your YMCA ID card.

Conditions:

  • This special YMCA membership rate is only available to students enrolled at Grand Valley State University for the semester specified at the top of this form.
  • Under this arrangement, GVSU students may opt in/out on a semester by semester basis without incurring a joining fee.
  • Membership is effective from the first day until the last day of the semester.
  • All membership inquiries, including joining after the on-line enrollment period has concluded, and other membership-related requests should be directed to the membership services contact at the David D.   • All changes and/or related financial adjustments will be handled through the office of the Membership Director at the David D. Hunting YMCA, 475 Lake Michigan, NW, Grand Rapids, 49504.
  • Parking privileges at the David D. Hunting YMCA are NOT INCLUDED in this special membership package.
  • Participating locations include David D. Hunting (Grand Rapids), Visser Family (Grandville), Wolverine Worldwide (Belmont), Southeast (Cascade), Lowell, Ionia, and the new Spartan Stores YMCA (Wyoming).
