Application Materials Checklist

**We are no longer accepting applications for the 2012-2013 academic year**

Please submit the following to complete your application packet:

• Completed online Application Form by January 27, 2012
• Applied for financial aid using a FAFSA Form by March 1, 2012
• Contact a GVSU professor to serve as your referee(person to write your recommendation and have them submit it to Courtney Sherwood at sherwoco@gsvu.edu or to B-4-232 MAK)
• Completed the Plan of Study Form showing what courses you plan to take and when you plan to take them by January 27,, 2012 and emailed to sherwoco@gvsu.edu or delivered to B-4-232 MAK.
• A written personal statement emailed to sherwoco@gvsu.edu that explains your academic and professional goals by January 27, 2012.
