School's out for the summer... Now what? The lazy, hazy days of summer require some additional planning for parents of school-age children.

Work Place Options, part of GVSU's employee assistance service, is able to help you find a summer day care program or camp for your child. Simply call 1-800-952-3586 and press number 4. Work Place Options will do a tailored search for summer care and save you a great deal of time!

GVSU Health & Wellness also has some suggestions and ideas on different camps from all over Michigan, and summer daycare programs for your children!

2011 W2 Information Box 12 Code W

2011 was the first year GVSU offered the HSA plan for Health Insurance. Per IRS regulations we must report the amount you contributed pre-tax through your payroll deductions AND the amount GVSU contributed (also pre-tax), these amounts are added together and reported in Box 12 Code W. This may be confusing when preparing your tax returns due to the fact that the code is labeled “Employer’s Contributions” which can be misleading. The IRS detailed instructions require us to report both the employer and employee amounts using the same code and they refer to the total as the employer contributions.

If you have an HSA you will be required to fill out the IRS form 8889. Please see the link below to the form 8889 and the corresponding instructions. If you have any questions while filling out this form please consult with your tax accountant or review the IRS instructions very carefully.

To view your payroll deductions log into My Banner Self-Service, Employee Tab, Pay Information, Deduction History. Request the time period you wish to see your deductions for and the amounts will be listed. Information listed here may also be helpful to you while preparing your personal tax returns.

New GVSU 403(b) and 457(b) Retirement Plans investment fund options

Effective February 1, 2012, Fidelity Investments and TIAA-CREF will introduce important changes to the GVSU 403(b) and 457(b) retirement plans. Your retirement plan will be enhanced with new investment choices, including a brokerage account and lower investment fees.
