Description

It’s estimated that 1 out of every 3 American adults have prediabetes. Without intervention, up to 30% of these individuals will develop type 2 diabetes with 5 years.

GVSU is partnering with Priority Health and the National Kidney Foundation to offer the CDC’s National Diabetes Prevention Program, at no cost to eligible participants. This program is a proven way to reduce an individual’s risk of developing Type 2 diabetes by up to 58%*.

The program works by giving the information and support needed for participants to make lifestyle changes including:

• Increased physical activity
• 5 to 7% weight reduction throughout the duration of the program
• Support to identify and overcome barriers to achieving health related goals Led by a lifestyle coach from the National Kidney Foundation of Michigan, the program teaches participants about things like nutrition and stress reduction.

Participants also learn how to track eating and exercise, and collaborate to overcome obstacles.

Eligibility
If you’re a Priority Health member with a body mass index (BMI) over 25 and you’re diagnosed with pre-diabetes, you can sign up for this free program. Not sure if you’re eligible? Take this quiz to find out: DoIHavePrediabetes.org.


Session Information
The diabetes prevention program meets one hour a week for 16 weeks and then monthly for 6 to 8 months. The program will be held at the Grand Valley State University Pew campus. Registration
Please call the National Kidney Foundation of Michigan at their Grand Rapids location at 616.458.9520 to register for the information session and learn more about the program. Participants who complete 9 of 16 classes can receive credit for the Engage with Health Coach component of the Healthy Choices wellness program, in lieu of connecting with a health coach.
