Birth, Adoption or Placement for Adoption

Birth of Child

Things to consider:

  • Notify the Benefits Office within 30 days of the birth of the baby
  • Enroll newborn into GVSU Benefit Plans
    • Email the Health and Wellness Office at healthandwellness@gvsu.edu with the following information within 30 days of the childs birth:

      Your G Number, Name of child, Date of Birth, Sex of child, Social security number, if the child have any other coverage, the type of insurance you would like the child added to (medical, dental, etc.), and Primary care physician if you have Priority Health HMO plan

  • Send in Social Security information once card is received
  • Review FMLA policy

Adoption

Things to consider:

  • Notify the Benefits Office within 30 days of the date of Birth
  • Enroll dependent(s) into GVSU Benefit Plans
    • Complete correct Medical Plan Enrollment form
    • Return form to HR within 30 days of the Birth
    • Bring a copy of the court order for the adoption

You may also want to:

  • Submit form for adoption reimbursement for up to $3000 for eligible expenses related to adoption
  • Enroll your dependent into the Dependent Life Insurance Program
  • Add/change life Insurance beneficiary
  • Change beneficiary with retirement vendor
    • TIAA/CREF
    • Fidelity
    • Hourly Staff Defined Benefit Pension Program
  • Increase Flexible Spending account(s)
  • Complete FSA enrollment form
  • Return form to HR within 30 days of the adoption

Placement of a child - Guardianship or Foster Care

Things to consider:

  • Notify the Benefits Office within 30 days of the date of Birth
  • Enroll dependent(s) into GVSU Benefit Plans
  • Complete the proper Medical Plan Enrollment form
    • Return form to HR within 30 days of the Birth
    • Bring a copy of the court order for the adoption

  • Add/change life Insurance beneficiary
  • Change beneficiary with retirement vendor
    • TIAA/CREF
    • New England Financial
    • Fidelity
    • Hourly Staff Defined Benefit Pension Program
  • Increase Flexible Spending account(s)
  • Complete FSA enrollment form
  • Return form to HR within 30 days of placement
