Assistantship Forms

The Graduate Assistantship appointment form (also referred to as "GA contracts") has been updated and is now a fill-in PDF form.  Please do not use the old version of the form under any circumstances. If you do so, we  may return them to you and ask you to redo the appointments using the new form.

This same form should be used for all academic year and by-semester appointments.

• Please be sure to indicate the GA's start and end dates and use the drop down arrow to select the beginning and ending payroll dates. The beginning payroll date is the closest date BEFORE the GA's start date, and the ending payroll date is the closest date AFTER the GA's end date.
• The GA should thoroughly review the contract and be familiar with the terms and conditions therein, prior to signing.
• The updated form posted here has been updated to allow the user to save a copy with the filled-in data intact.
