Registration forms:

Registration Permit Request

Step 1: Download the permit and then fill out the top section with your contact information, the course information, and a brief project description if applicable.

Step 2: Obtain either the signature of the faculty member who you are working with OR en e-mail from them granting you permission into their class.

Step 3: Drop the permit off (with signature / e-mail attached) to the Biology Department (212 Henry Hall), e-mail to biodept@gvsu.edu, or fax to (616) 331-3446.


Prerequisite Waiver Request

Step 1: Download the permit and then fill out the top section with your contact information and the course information. Please also check one of the boxes at the top of the sheet indicating why you are trying to obtain the waiver.  

Step 2: Obtain either the signature of the faculty member who is teaching the class OR en e-mail from them granting you permission into their class.

Step 3: Drop the permit off (with signature / e-mail attached) to the Biology Department (212 Henry Hall), e-mail to biodept@gvsu.edu, or fax to (616) 331-3446.


Closed Class Permit Request

Step 1: Download the permit and then fill out the top section with your contact information and the course information.

Step 2: Obtain either the signature of the faculty member who is teaching the class OR en e-mail from them granting you permission into their class.

Step 3: Drop the permit off (with signature / e-mail attached) to the Biology Department (212 Henry Hall), e-mail to biodept@gvsu.edu, or fax to (616) 331-3446.

Note: All permits need to be signed by the Faculty member teaching the class before they can be turned into the Department. If the faculty member is not available to sign the permit, the department will also accept an email or note from them granting the student permission into the class. Permits may either be dropped off in the Biology Department (212 Henry Hall), emailed to biodept@gvsu.edu, or faxed to 331-3446. Permits received will be processed within 2 business days.
