Written plans for the proposed flexible work arrangement should be submitted to your immediate supervisor and will require approval and signatures from both your immediate supervisor and appointing officer. This will help to facilitate understanding between all involved parties. Communication is critical to the success of a FWA. The written plan document will also provide an essential key to refer back to during the course of the FWA.
For hourly staff, supervisors must notify payroll of the FWA, by email (firstname.lastname@example.org) or by phone, on the Wednesday prior to the proposed starting date at the latest. For all staff, the application form for a FWA should be submitted to Human Resources, 1090 JHZ. This form will be housed in the Work Life Connections Office as a tool to track FWA’s usage.
