GVSU Family Health Center
Travel Health and Immunizations
Our office offers routine immunizations in addition to immunizations needed for travel:
Hepatitis A & B
(Certified Yellow Fever Vaccination Clinic)
We can also provide malaria prophylaxis prescriptions for patients that do not have a primary care provider to do so. For this, we require an appointment with a nurse practitioner.
You may print off paperwork for your first appointment and travel immunization appointments through the following links:
To determine which immunizations you need based on where you are traveling to, please visit the Centers for Disease Control (CDC) website at http://wwwnc.cdc.gov/travel/destinations/list.aspx.
If you are unsure of what routine immunizations you have had, contact your primary care provider to obtain an immunization record. If you are not able to obtain your immunization record through your PCP you may set up an appointment with one of our nurses to assist you.
For information on any immunizations (routine or travel), please visit the Michigan Department of Community Health website at http://www.michigan.gov/mdch/0,1607,7-132-2942_4911_4914-138197--,00.html to view their Vaccine Information Sheets.
*Please note that if you are on the sliding scale for payments, your reduced payments do not apply to immunizations.
