Know Your Numbers is back on campus March 12-16 2012!

There are 3 ways you can complete the biometric component of Know Your Numbers

1. Participate on site: Sign Up Online or call 866.935.5462

2. Participate off site: See instructions below.

3. Use your annual physical with your own health care
professional: Click here.

REMEMBER! Regardless of which method you select to complete the biometric screen you must complete the online Health Risk Assessment between March 2 and March 23 2012 to qualify for Healthy Choices incentives.

2012 Remote/Offsite Schedule
If you are unable to make an on site appointment or work on the Holland and Muskegon or Traverse City regional centers, off site locations are available through the Remote Testing Program offered by Wellness Inc. Simply ask when making your appointment.

Remote Testing Dates:
Start Date: Friday, March 2nd
Fax in Date: Friday, March 16th (last date to accept fax from participant for program)
Lab Date: Friday, March 23rd (last date to visit lab)
