2. Participate off site: See instructions below.\r\n
3. Use your annual physical with your own health care\r\n

REMEMBER! Regardless of which method you select to complete the biometric screen you must complete the online Health Risk Assessment between March 2 and March 23 2012 to qualify for Healthy Choices incentives.\r\n

If you are unable to make an on site appointment or work on the Holland and Muskegon or Traverse City regional centers,  off site locations are available through the Remote Testing Program offered by Wellness Inc. Simply ask  when making your appointment.\r\n
