Test Accommodation Form
Student: It is YOUR RESPONSIBILITY to work with your professor, complete this form, and submit it AT LEAST ONE WEEK BEFORE the scheduled time of your exam. This timeframe is necessary so arrangements can be made for testing rooms and proctors. Use this form only if you are requesting testing accommodations through Disability Support Services. If your instructor provides testing accommodations, you do not need to use this form. (Please review your syllabi and discuss dates and times for ALL of your exams for the semester with your Instructor.) This form must be completely filled in.

NOTE: You and your instructor should agree upon the date and/or time of your exams to be taken through DSS.
Your instructor must approve any subsequent changes. The DSS Office is open for testing Monday through Thursday,
8 am to 9 pm, and on Friday 8 am to 5 pm. All exams must be scheduled and completed during these hours. You may not
stop working on your exam and return at a later time and/or day to complete the remaining unfinished exam without your
instructor's written approval. Arrive at the DSS Office at the scheduled time of your exam. We may not be able to
accommodate you if you arrive late.
